DR Goalkeeper Academy

Waiver & Consent
As the parent/guardian of the child who is named in this DR Academy Registration application, I acknowledge that participation in the goalkeeping academy/camp activities involves inherent risks of injury. I understand that the academy and its staff will take all reasonable precautions to ensure my child's safety, but I voluntarily assume all risks and hazards associated with their participation. In consideration for allowing my child to enrol and participate in the academy, I hereby release and discharge the academy, its officers, employees, and agents from any and all liability for injuries, damages, or losses that may occur as a result of my child's participation.

I also agree to ensure that my child abides by all rules and regulations set forth by the academy. I understand that this waiver is legally binding, and I have read and understood its contents. I willingly signed this waiver as a condition of enrolling my child in the goalkeeping academy/camp. We seek your permission to use photographs and videos of your child, who is named in this DR Academy Registration application, taken during their participation in our academy and camp activities. These images may be used for promotional and marketing purposes, both online and in printed materials.
